Density versus Quality in Health Care Provision: Using Household Data to Make Budgetary Choices in Ethiopia 总被引:1,自引:0,他引:1
Usage of health facilities in Ethiopia is among the lowest inthe world; raising usage rates is probably critical for improvinghealth outcomes. The government has diagnosed the principalproblem as the lack of primary health facilities and is devotinga large share of the health budget to building more facilities.But household data suggest that usage of health facilities issensitive not just to the distance to the nearest facility butalso to the quality of health care provided. If the qualityof weak facilities were raised to that currently provided bythe majority of facilities in Ethiopia, usage would rise significantly.National data suggest that given the current density and qualityof service provision, additional expenditure on improving thequality of service delivery will be more cost-effective thanincreasing the density of service provision. The budget allocationrule presented in the article can help local policymakers makedecisions about how to allocate funds between improving thequality of care and decreasing the distance to the nearest healthcare facility. 相似文献

Firm level data for the manufacturing sector in Africa, presented in this paper, shows very low levels of investment. The importance of profit effects on investment is investigated using a flexible accelerator, a specification based on the Euler equation and a simple generalisation of these specificiations. There are controls for firm fixed effects. It is shown that the profit effect is very similar for both the accelerator and Euler equation specifications. A comparison with other studies shows that, for small firms, the effect is much smaller in Africa than for other countries. Reasons for the relative insensitivity of investment to profits in African firms are suggested. For the most general specification tested there are no significant differences in the size of the profit effect across the four countries in the study. 相似文献

We investigate the question of whether firms in Africa's manufacturingsector are credit constrained. The fact that few firms obtaincredit is not sufficient to prove constraints, since certainfirms may not have a demand for credit while others may be refusedcredit as part of profit maximising behaviour by banks. To investigatethis question, we use direct evidence on whether firms had ademand of credit and whether their demand was satisfied in theformal credit market, based on panel data on firms in the manufacturingsector from six African countries. Of those firms with a demandfor credit, only a quarter obtained a formal sector loan. Ouranalysis suggests that while banks allocate credit on the basisof expected profits, micro or small firms are much less likelyto get a loan than large firms. We also find that outstandingdebt is positively related with obtaining further lending. Therole of outstanding debt is likely to be a reflection of inefficiencyin credit markets, while the fact that size matters is consistentwith a bias as well, although we cannot totally exclude thatthey reflect transactions costs on the part of banks. Most risk-sharing tests on developing country data are conducted at the level of the village; generally, the full risk-sharing hypothesis is rejected. This paper uses detailed data on all insurance networks within a village in Tanzania; networks are not clustered but largely overlapping. We test whether full risk-sharing occurs within these networks. While village level full-insurance cannot be rejected for food consumption, we find evidence consistent with at least partial insurance of non-food consumption via networks. 相似文献

Are markets in developing and transition economies over‐regulated or under‐regulated? This is a perennial question in the development discourse, but one for which answers appear to come more from preset ideology than from context‐specific analysis and evidence. These issues become even more pressing when the debate turns to the links between regulation, deregulation and inequality. The recent experience of rising inequality in many countries has also brought to the fore predictable policy positions. A key aspect of labor regulation in developing countries, and one becoming more prominent in the era of rising inequality, is the minimum wage. The range of issues around regulation, minimum wage and informality was addressed by papers presented to a conference held in New Delhi on December 17–18, 2014. The conference was co‐sponsored by the World Bank, UK Department for International Development, Cornell University and Columbia University. The organizers of the conference were Kaushik Basu, Stefan Dercon, Ravi Kanbur and Jan Svejnar. A selection of papers from the conference which passed the usual review procedures of the Review of Development Economics, and a further selection of papers from those submitted to the journal, form this symposium. 相似文献

This article develops a method for targeting antipoverty programsand public projects to poor communities in rural and urban areas.The method calls for constructing an extensive data set froma large number of sources and then integrating the entire setinto a geographic information system. The data set includesdemographic data from the population census; household-leveldata from a variety of surveys; community-level data on localroad infrastructure, public facilities, water points, and soon; and department-level data on agroclimatic conditions. Aneconometric model that estimates the impact of household-, community-,and department-level variables on household consumption is usedto identify the key explanatory variables that determine thestandard of living in rural and urban areas. This model is thenapplied to predict poverty indicators for 3,871 rural and urbancommunities in Burkina Faso and to map the spatial distributionof poverty in the country. A simulation analysis assesses theeffectiveness of village-level targeting based on these predictions.The results show that such targeting is an improvement overregional targeting in that it reduces leakage and undercoverage. 相似文献
