我国城乡医疗救助统筹管理的适宜模式研究

在全面梳理和总结各地城乡医疗救助统筹管理试点经验的基础上,将当前城乡医疗救助统筹管理归纳为"制度及资金账户完全一体化"、"账户分立,但资金可调剂"、"制度统一,但账户仍分立"、"结合城乡医疗保障一体化实现统筹"等四种模式,并对各种模式做出了评价。提出医疗救助城乡统筹可分为资金和实施两个层面,以及广度、深度和宽度三个维度。医疗救助要围绕救助资金这个核心来设计和推行,根据各地的社会经济发展情况选择逐步推进还是全面推进的实施路径,并通过提高经济水平、改善制度环境、利用社会资源、完善信息基础、推进一站式服务等措施来促进城乡医疗救助制度的融合和统筹管理。

Study on the Suitable Integration Model of Urban and Rural Medical Assistance in China

Based on the analysis on how to integrate the urban and rural medical assistance in different pilot areas,four models were summarized and assessed.They were"Both of the systems and accounts were integrated","The accounts were separated,but the funds were Adjustable","The systems were integrated,but the accounts were separated","Combine with the Basic Medical Security to achieve urban-rural integration".The integration of urban and rural medical assistance could be divided into two level and three dimensions,while the funds integration was the core.A stepwise or comprehensive implementation path should be chosen based on current socioeconomic status of different areas.Such additional measures as enhancing the economic level,improving the institutional environment,utilizing the social resources,perfecting the information platform and promoting the one-stop service should be implemented to promote the integration of urban and rural medical assistance system.
