盐酸米诺环素联合Er,Cr:YSGG激光治疗慢性牙周炎的临床效果及对患者炎症介质的影响

目的探讨盐酸米诺环素联合Er,Cr∶YSGG激光治疗慢性牙周炎的临床效果及对患者炎症介质的影响。方法选取2019年1月至2020年6月鞍山市中心医院收治的84例慢性牙周炎患者作为研究对象,采用随机数字表法分为对照组与观察组,各42例。对照组予以龈上洁治、龈下刮治与盐酸米诺环素治疗,观察组42例加用Er,Cr∶YSGG激光治疗,比较两组治疗效果。结果治疗后,观察组治疗有效率95.24%,高于对照组的76.19%,差异有统计学意义(P<0.05)。观察组治疗后牙龈指数(GI)、牙周探诊深度(PD)、菌斑指数(PLI)、龈沟出血指数(SBI)改善情况优于对照组,差异有统计学意义(P<0.05)。治疗后两组肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-1β(IL-1β)水平较治疗前降低,且观察组各指标低于对照组,差异有统计学意义(P<0.05)。结论慢性牙周病治疗中,将盐酸米诺环素与Er,Cr∶YSGG激光联合应用,可显著改善患者牙周状况,减轻炎症反应,临床疗效确切。

Clinical Effect of Minocycline Hydrochloride Combined with Er,Cr:YSGG Laser in the Treatment of Chronic Periodontitis and Its Influence on Inflammatory Factors

Objective To explore the effect of minocycline hydrochloride combined with Er,Cr:YSGG laser in the treatment of chronic periodontitis and its influence on patients with inflammatory mediators.Methods A total of 84 patients with chronic periodontitis admitted to Anshan Central Hospital from January 2019 to June 2020 were selected as the research objects and were divided into a control group and an observation group by random number table method,with 42 cases in each group.The control group were treated with supragingival scaling,subgingival scaling and minocycline hydrochloride.Patients in the observation group were treated with Er,Cr:YsGG laser,and the therapeutic effects of the 2 groups were compared.Results After treatment,the total effective rate of the observation group was 95.24%,higher than that of the control group(76.19%),the difference was statistically significant(P<0.05).The improvement of gingival index(GI),periodontal probing depth(PD),plaque index(PLI)and sulcus bleeding index(SBI)in the observation group was better than that in the control group,with significant differences(P<0.05).After treatment,the levels of tumor necrosis factor(TNF-),hypersensitive C-reactive protein(hs-CRP)and interleukin-1β(IL-1β)in the 2 groups were all lower than those before treatment,and the indexes in the study group were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion In the treatment of chronic periodontal disease,minocycline hydrochloride combined with Er,Cr:YsGG laser can significantly improve the periodontal status of patients,and reduce the inflammatory response of patients,with accurate clinical efficacy.
