躯体的治愈并不能令患者得到完全的健康,心身的整体健康才是医本之源。近年来,北京市丰台区铁营医院(下称"铁营医院")的治愈率不断攀升,患者满意度也越来越高,铁营医院院长孙培云表示:"这都源于心身医学的发展。"由于很多心理及社会因素所产生的疾病均是以躯体化形式呈现,单纯治疗患者的躯体疾病,很难使疾病达到根本治愈。作为二级医院,没有三级医院人满为患的门诊量,有足够的时间筛查患者,又超越了社区医院的医疗技术,能有效在各科室开展心身医学的介入,严格把控医疗质量。"由于患者对心身疾病不了解及病耻感等原因,很多心身疾病的患者会首先到我们这样的二级医院就诊。如能有效识别和治疗这部分患者,将极大地减轻三级医院的诊疗负担。"孙培云如是说。  相似文献

Recent developments in medical technology and bioethics have led to a shift in the traditional framework of physician–patient responsibility in health care decision making. Patients are increasingly participating in serious health care decisions, resulting in a “shared decision-making model“ that both patients and their doctors agree is a significant improvement over the traditional system in which a doctor served as the primary decision maker. However, patients are finding they need to develop strategies that allow them to understand and evaluate their health care options in a clear manner. T.L. Saaty’s “Analytic Hierarchy Process” (AHP) is a decision methodology that has been successfully applied to a wide variety of situations and has great potential as a methodology for improved shared health care decision making. This paper reports the results of an experimental use of AHP as a tool to facilitate shared decision making for two specific health care populations. Both groups of patients were deciding amongst their options in terms of two important elective health care procedures: estrogen replacement therapy and cosmetic eyelid surgery. After using AHP, the majority of both patients and physicians agreed that this technique improved physician–patient communication, thus greatly assisting shared health care decisions. When considering other criteria in addition to physician–patient communication, including the entire physician–patient relationship, the patients felt that AHP was preferable to the conventional doctor-patient mode of decision making.  相似文献

In summary, it is the writer's opinion that a healthy bottom line and quality patient care can coexist. With today's economic pressures and with more and tighter controls and regulations being imposed by the government and regulatory agencies, providers (especially hospitals) need to be cost efficient and cost effective. Will the cost-containment measures necessary to maintain a healthy bottom line reduce the quality of patient care? They needn't. In point, such measures might actually increase the quality of care by avoiding wasteful clinical practices. Quality patient care comprises three basic elements: value, service, and compassion. To omit any one of these ingredients is to remove the quality from patient care. Should this happen, no cost-containment measure will be enough to sustain a hospital. Financial accountability is essential; quality care is imperative. Blended together thoroughly, they are the recipe to survival--for the hospital and for the patient.  相似文献

Professional work such as nursing has traditionally been examined as being localized in the individual’s body where professional know-how and skills are residing in the cognitive faculties and in embodied action. Contrary to such a view, the concept of agencement, recently used in the social study of finance, underlines that agency is in the contemporary technoscientifically determined times of necessity distributed and includes a variety of tempospatially distributed resources. Reporting a study nursing work in a leukemia ward in a Swedish regional hospital, it is demonstrated that the conventional view of nursing as primarily being bedside care is only accommodating a subset of the totality of the nurses’ work. In addition to face-to-face care and patient interaction, nursing work is the mobilization of a great number of actors with different domains of expertise to safeguard the health care status of the patient. Speaking of nursing work as agencement is opening up for alternative and more accurate understandings of nursing work in an increasingly technologically determined health care system.  相似文献

目的:探讨护理管理在小儿康复病房中的作用并进行分析。方法:调查我院2010年3月起开展系统护理管理以来160例小儿康复患者的满意程度。结果:通过对系统管理的实施,发现在这160例患者当中,不曾出现一例纠纷问题,同时在患者出院后,病患家属的满意程度也达到93.75%,满意人数达150例。结论:系统护理的实施具备很大的优势,同时还让护理病房得到了全面的、系统的,以及详细的管理方法,从而将护理人员的工作热情和护理技术提高了上去,降低风险几率。  相似文献

This paper examines the mediating effects of social identification on the relationship between high performance work systems (HPWS) and psychological empowerment and the direct relationship between psychological empowerment and clinician perceptions of quality of patient care. We use structural equation modelling on a sample of 254 health professionals from a large regional hospital in Australia. Results demonstrate, first, a strong effect of HPWS on social identification. Second, social identification mediates the relationship between HPWS and psychological empowerment and, third, psychological empowerment has a strong effect on clinician perceptions of quality of patient care. Implications are drawn for management theory and hospital managers.  相似文献
