Objective: To evaluate cost-effectiveness of brentuximab vedotin in patients with relapsed/refractory Hodgkin lymphoma who have received autologous stem cell transplantation, from a Scottish healthcare payer perspective.
Methods: A Microsoft Excel-based partitioned survival model comprising three health states (progression-free survival [PFS], post-progression survival, and death) was developed. Relevant comparators were chemotherapy with or without radiotherapy (C/R) and C/R with intent to allogeneic hematopoietic stem cell transplantation (alloSCT). Data were obtained from the pivotal phase II single-arm trial in 102 patients (SG035-0003; NCT00848926), a systematic literature review and clinical expert opinions (where empirical evidence was unavailable). PFS and overall survival for brentuximab vedotin were estimated using 5-year follow-up data from SG035-0003, and extrapolated using event rates observed for comparator treatments from published survival data. Resource use included drug acquisition and administration; alloSCT; treatment of adverse events; and long-term follow-up. Deterministic and probabilistic sensitivity analyses were conducted to evaluate the impact of uncertainty.
Results: In the base case, the incremental cost-effectiveness ratio (ICER) for brentuximab vedotin was £38,769 per quality-adjusted life year (QALY) vs C/R, whereas C/R with intent to alloSCT was dominated by brentuximab vedotin. ICERs for brentuximab vedotin generated by the deterministic sensitivity analysis ranged between £32,000–£54,000 per QALY. Including productivity benefits reduced the ICER to £28,881 per QALY.
Limitations: Limitations include lack of comparative data from this single arm study and the heterogeneous population. Inconsistent baseline characteristic reporting across studies prevented complete assessment of heterogeneity and the extent of potential bias in clinical and cost-effectiveness estimates.
Conclusions: Although the base case ICER is above the threshold usually applied in Scotland, it is relatively low compared with other orphan drugs, and lower than the ICER generated using a previous data cut of SG035-0003 that informed a positive recommendation from the Scottish Medicines Consortium, under its decision-making framework for assessment of ultra-orphan medicines. 相似文献
This study examines the relationship between student perceptions of different types of educator power and different modes of student complaining behaviour in the case of university education. A large sample of marketing students in the business school responded to the study from a state university in Northeastern United States. Factor analysis and canonical correlation analysis are used to explore the relationships between five bases of power perceptions (referent, expert, reward, legitimate, and punishment) and four modes of complaining behaviour (voice, negative word of mouth, third party, and exit). The results indicate that students engage in different modes of complaining as they perceive different types of educator power. The predominant complaining mode is found to be voice under referent or expert power, third party under legitimate power, and exit under reward or punishment power. Our findings offer important implications for student satisfaction, retention, and completion rates in higher education. 相似文献
As this historic presidential election draws near, what do RNs think of our nation's priorities? Which candidates do they think will be most effective in shaping our health care system and addressing the most pressing issues of our time? The results of this survey show that RNs do not identify overwhelmingly with one political ideology or party and, in fact, they closely resemble the public on these political dimensions. The data also show that RNs identify health care issues as the most important problem facing the nation. RNs who believe that it is the responsibility of the government to provide health insurance to those without it, have more confidence in the government to achieve this outcome, and are more likely to be Democrats than Republicans. The presidential election is expected to be highly contested and could be determined by a relatively small margin of votes and, thus, nurses should recognize their chances of influencing the outcome of the election. The data from this survey provide baseline information potentially useful to increasing the political influence of the nursing profession, informing other organizations about where they might align with nurses, and helping candidates and the political parties compete more effectively in seeking the support of roughly 3 million RN voters. 相似文献
County officials in the seven-county metropolitan area of the State of Minnesota's Twin Cities of Minneapolis and St Paul were seeking to redefine county roles in service delivery. This article reports on a process model that was piloted on two services. The model was commissioned by officials; was based upon and informed by literature and comparative experience; began with an examination of functions performed; involved participants in focusing the study; was context-focused; and involved local stakeholders in projecting the implications of alternative county roles. These characteristics complicated the research management process because the process took more time, involved more actors, and required more coordination than traditional research. However, having stakeholders identify alternatives and bring factual material to the process was critical to the success of the model and including the comparative and contextual research both strengthened the policy conclusions and improved the probability of their adoption and implementation. A case of a Minnesota county that followed the model in an effort to reform water governance within the county illustrated adoption of the approach. The stakeholder analysis process was previously applied in Africa, China and Southeast Asia and the model should be applicable to a wide range of settings and problems. 相似文献
Objective: Philadelphia chromosome negative [Ph(?)] relapsed or refractory (R/R) B-precursor acute lymphoblastic leukemia (ALL) is an extremely rare condition requiring intensive treatment. This retrospective chart review aimed to quantify hospitalizations and reimbursement in this patient population in France.Methods: Patients aged ≥18 years and with at least one hospitalization for Ph(?) R/R B-precursor ALL were included in the study. They were relapsed with first remission lasting <12 months, relapsed after first salvage therapy, relapsed any time after hematopoietic stem cell transplant (HSCT), or were refractory to initial or salvage therapy. Data were collected from the index date (first diagnosis of R/R ALL) until death or loss to follow-up. The chemotherapy period was defined as the first chemotherapy date after the index date to the earliest of death, loss to follow-up, last chemotherapy dose plus 30 days, or initiation of HSCT. The primary outcome was the percentage of time hospitalized during the chemotherapy period.Results: Thirty-three patients were included, with a mean age of 49 years. The mean proportion of time spent in the hospital during the chemotherapy period was 46% (95% CI =34–57%). Patients had a mean of 2.2 (SD =1.5) inpatient hospitalizations and the mean length of stay per hospitalization was 16.8 (SD =14.8) days. During the chemotherapy period, the mean amount reimbursed per hospitalization was €31 067 (SD = €4850) and the total hospitalization reimbursement per patient was €68 344. From the index date to death, excluding HSCT, the total reimbursement per patient was €108 873.Limitations: The sample size was small, although this was expected given the rarity of the patient population.Conclusions: Adults with Ph(?) R/R B-precursor ALL had repeated and prolonged hospitalizations during salvage chemotherapy. Approximately half the follow-up period was spent in the hospital, and this time was associated with high economic burden in France. 相似文献
This research was designed to investigate the effects of organizational citizenship behaviors (OCBs) and objective sales productivity on sales managers’ evaluations of their sales personnel’s performance and to examine whether the impact of OCBs on performance evaluations is greater at higher levels of the sales organization hierarchy. Two samples were obtained from the same organization: a sample of 987 multiline insurance agents and a sample of 161 agency managers. Objective measures of sales productivity were obtained for both samples along with evaluations of three dimensions of OCBs and an assessment of overall performance. The results indicate that managers’ evaluations are determined at least as much by OCBs as they are by objective measures of performance. After partialing out common method variance, the results also indicate that OCBs account for a greater proportion of a sales manager’s evaluation than of a sales representative’s evaluation. ABSTRACTThis study delivers a clearer understanding of the constitution of the datapreneurial consumer, the role of the market in that construction, and the implications for consumer identity projects in the age of Big Data and an increasingly data- and surveillance-driven society. The study uses a theoretical framework of the “quantified self” (QS) to examine consumers (re)building creditworthiness. In the context of a major online credit-user forum, it employs creative-nonfiction methodology to protect forum-member privacy. To the literature on creditworthiness, the study contributes a process model of the construction of the datapreneurial credit consumer identity. To the QS literature, it offers insight into how consumers may embrace quantification and self-tracking, even in areas where they are nudged or pushed into it. To the sociology of quantification literature, it adds empirics to explain how consumers may embrace market-provided self-quantification resources in attempts to liberate themselves from the structural control of that very quantification. 相似文献
The importance of communication for the successful development of new projects, particularly within the R&D laboratory setting, has been well documented. Yet researchers have seldom examined the relationship between patterns of communication and cross-functional cooperation in the development and management of new programs. In this article Mary Beth and Jeffrey Pinto report on the results of a research study that assessed the relationship of two aspects of project team communication (formal versus informal modes and reason for communication) with the level of cross-functional cooperation actually achieved within a hospital project team charged with developing a new program. A total of 262 team members were surveyed from 72 hospital project teams. The results demonstrated that high cooperation teams differed from low cooperation teams both in terms of their increased use of informal methods for communication as well as their reasons for communicating. Finally, cross-functional cooperation was found to be a strong predictor of certain project outcomes. 相似文献
