Managed Mental Health Care: Stakeholder Effects and Policy Implications

ABSTRACT: During the past several years, healthcare delivery in the United States has shifted from a primarily fee-for-service system toward managed care. Mental health care has been affected disproportionately by this trend. The existing literature on managed mental health care is abundant, but narrowly focused. The purpose of this study is to take a broader public-policy approach, considering simultaneously the effects on system stakeholders: patients, providers, employers, insurers, and society.     

Mitigating America's demographic dilemma by pre-funding social security

Financing Social Security benefits at current levels implies significant increases in payroll taxes within the next 20 years under current US demographic developments. Using a general-equilibrium overlapping-generations model with realistic patterns of fertility and lifespan extension, this study shows that future generations would be harmed during the demographic transition due to rising payroll taxes, which crowd out savings and slow real wage growth below the rate of technological progress. A faster rate of technological progress would mitigate only some of the payroll tax increase and its economic consequences but could not overcome them. Addressing the financing problem by reducing Social Security benefits as needed or by raising the eligibility age for benefits imposes major welfare losses on current or near term retirees. By contrast, a pre-funding of Social Security financed with consumption taxes more evenly spreads the welfare losses across generations, and it helps future generations, especially the poor, by stimulating capital formation.

Top dogs have set the agenda for peace: international social position and peace attitudes

Everybody agrees that peace is good, but not on how to achieve it. The Y2K study contained many questions on peace proposals, as well as on prospects for East/West and North/South peace. It ranked these proposals by popularity and studied how the social positions of the 1967 respondents and their countries affected what proposals were preferred and their predictions on peace or war. It now turns out that, neither centre, nor periphery got it quite right. In the former East, the people set the agenda and removed the iron curtain. In the West, however, the top dogs set the agenda, with military intervention, even called ‘peace enforcement’, high on it. Recent political and military developments indicate that ‘peace’ is likely to remain ‘peace between top dogs’; there will be few advocates for putting severe conflicts and violations of human rights in the periphery (e.g. Burundi and Sudan) high on the peace agenda.
