本文利用分类变量的数据包络分析法(DEA with Categorical Vailables)和数据包络分析法的产出导向模型(BCC模型)对我国26个省市的392家城市二级以上医院2004年和2005年的经营效率进行了分析与比较。分类DEA模型的研究结果表明.不同等级医院之间存在效率差异:高等级医院效率反而最低,低等级医院效率反而最高,这个结论与BCC模型得出的结论完全不同。这一结论揭示出我国医院经营中存在的两个制度性问题:一是我国医院等级评审制度的无效性;二是医院在经营过程中存在严重的资源浪费。然而.对于不同地区医院效率的评价,两种模型结果相似:东部地区医院经营效率高于中部和西部地区。 相似文献

Marketing and environmental psychology studies have long emphasized the importance of signage in large and dispersive service settings but have focused little on utilitarian service contexts. Previous studies have also analyzed the role of emotions in customer satisfaction with the servicescape, particularly in hedonic service environments such as malls, hotels, and restaurants, but a limited research has been conducted on the role of emotions in utilitarian service settings such as healthcare services. This study draws from the environmental psychology and service marketing literature to investigate the effects of signage and emotions on satisfaction with the servicescape in the hospital setting. In addition, it explores the moderating effect of emotions on the relationship between signage and satisfaction with the servicescape. The findings show that signage has a positive and significant effect on satisfaction with the servicescape. Negative emotions have a significant negative effect on satisfaction, while positive emotions have no significant effect. Differently from what was expected, emotions do not moderate the relationship between signage and satisfaction. Managerial implications are provided for service managers who wish to design a utilitarian service setting to effectively increase user satisfaction with the servicescape. 相似文献

How much does a dramatic increase in technology improve healthcare quality in an upper middle‐income country? Using rich vital statistics on infant health outcomes, this study evaluates the effect of introducing technologically advanced perinatal hospitals in 24 regions of Russia on infant mortality during the period 2009–2013. A 7‐year aggregate panel dataset reveals that opening a perinatal centre corresponds to infant mortality reduction by 3.8 percent from the baseline rate, neonatal (0–28 day) mortality by 7 percent and early neonatal (0–6 day) mortality by 7.3 percent. We find that the perinatal centres help to save 263 additional infant lives annually, ranging from 3 to 25 lives in regions with different birth rates. However, we further find that an average cost per life saved is 52 million rb (or 2.6 million 2014 PPP USD), which is much higher than the cost of similar interventions in the United States. 相似文献

Using administrative records of births from the Perinatal Surveillance System of the Peruvian Social Security System (ESSALUD), we test whether high admissions of pregnant women affected non-elective caesarean section (C-section) rates in the ESSALUD public hospitals during 2005–2006. We present a basic theoretical model that considers physician preferences for leisure and hospital capacity constraints, and test the model predictions. We find that physician demand for leisure increases the probability of C-sections in small and medium-size hospitals, while hospital capacity constraints set a limit on them. We discuss the policy implications as well as the policies implemented to avoid unnecessary C-sections. 相似文献

The present study investigates the relationship between organizational commitment and work–family conflict for medical doctors and nurses. Using canonical analysis on questionnaire, this relation has been tested distinctively through three sub-dimensions of organizational commitment listed: affective commitment, continuance commitment and normative commitment. Data collected from 766 (411 doctors and 355 nurses) individuals employed at seven state hospitals and three medical faculties of universities in Ankara. Analysis revealed that the relationship between organizational commitment scale and work–family conflict scale is weak and positive. Results suggest that the more committed the doctors and nurses are to the hospital they work for, the more work–family conflict they experience. On the basis of the sample information obtained in this study, it is possible to claim that if the employees are supported to establish a fair balance between their work and family lives in the guidance of these principles, a strong commitment to the organization will develop. 相似文献

In this paper a theoretical model is developed to identify the main determinants of the proportion of temporary workers at firms. The outcomes show that the proportion of temporary workers has a counter-cyclical behaviour: it grows during the slump period up to 1995 and falls during the subsequent recovery. However, given the effect of the general economic cycle, firms that raise their sales or improve their market dynamism index tend to increase their proportion of temporary workers. This proportion also rises when the average labour cost decreases, firm size increases, and the knowledge capital stock diminishes. 相似文献

This article examines the strategies that have been used by central government and by local NHS and social care organizations to overcome difficulties of service co-ordination. This study aims at investigating the burden and pattern of clothing related pillion riders' injuries and their helmet wearing pattern in Lahore, Pakistan. Data were collected for nine months (July 2013 to March 2014), reporting 13,248 motorcycle related injuries, out of which 8751 (66%) cases were related to the clothing related pillion riders' injuries. Out of these injuries 10.30% were severe injuries which can be prevented or reduced by proper safety measure, e.g. wearing helmet. It was also found that most of the fatalities were not wearing helmet. Injuries due to wearing of loose fitting clothes and absence of helmet are not uncommon in Pakistan and other underdeveloped parts of the world. Such accidents may be reduced by creating awareness and safety sense in the riders, where use of safe dress and helmet should be encouraged in order to avoid such fatal damages. 相似文献
