Health reform remains the most contentious policy issue in Canada. Medicare is subject to the same political forces that demand state retrenchment in other areas, but it has escaped wholesale reorganization because of the commitment of Canadians to the principles of medicare, because the provinces and the federal government remain locked in a battle about provincial autonomy, and because the existing system serves the interests of various professional groups including organized medicine. Nevertheless, reform and, especially, expansion of coverage is essential if the existing system is to be sustained. The most encouraging aspect of current debate is the recognition that we can look beyond the U.S. for reforms more consistent with the underlying values of Canadians than is the U.S. system. 相似文献

A primary goal of the 1996 Telecommunications Act isto encourage competition in long-distance telephonemarkets. Four years after passage of thislegislation, Bell Operating Companies (``BOCs') havebeen granted permission to offer long-distanceservices in only one state. German public transport services by bus are characterized by a regulatory framework that distinguishes between commercial and non-commercial services. Contrary to the apparent views of legislators, this paper shows that costs and revenues are not the only parameters determining whether or not operators are able to provide services in a commercially viable way.Apart from the local characteristics of the specific service, we show that the classification of services as commercial versus non-commercial is determined in large part by the public transport authorities that set minimum quality standards to be provided by operators. Our analysis shows that the authorities awarding the contracts in some cases affect market organization significantly depending on how they make use of this power. Furthermore, market organization differs substantially with respect to the awarding structure and the contractual relationships, thus creating a challenge for operators and authorities in an embryonic market. 相似文献

In this paper, we study the effects of tax-exempt debt on the supply of charity care of non-profit hospitals. We hypothesize that hospitals using tax-exempt rather than taxable debt are forced to provide higher levels of charity care as a condition for gaining access to the tax-exempt market. The study uses a panel of 189 California non-profit hospitals. Hospital uncompensated care is regressed on the level of uncompensated care by other hospitals in the market, lagged values of tax-exempt and taxable debt and other control variables.The magnitude of the tax-exempt bond subsidy has a positive effect on the flow of charity care and varies positively with the charity care provided by other hospitals. We conclude that subsidies provided by tax-exempt debt are an effective media to increase the supply of charity care by hospitals. Regulators can use competition between non-profit hospitals in order to enforce the desired behavior. 相似文献

解决中低收入人群的基本住房问题是实现社会公平的基本国策之一。为了有效地实施保障性安居工程,通过构建数学模型研究在财政补贴率相同的条件下,住房消费补贴与住房供给补贴的成交价格、成交量与社会福利情况,可以发现这两者之间存在较大的差别,前者的成交价格、成交量和社会福利都高于后者,但前者比后者的财政补贴额更多。如果提高财政补贴率,那么前者成交价格会不断提高,而后者成交价格会不断降低,成交量都会呈现扩大趋势。成交价格、成交量和社会福利不但与财政补贴率及其补贴方式有关,也与住房的需求弹性和供给弹性有关。研究结论的政策意义在于,如果在政府财力有限、住房价格上涨压力较大的条件下,应当采用住房供给补贴;如果在政府财力相对宽松、住房价格上涨压力相对较小、依靠住房拉动经济增长的压力较大的条件下,则应当采用住房消费补贴,同时它还能实现公平与效率兼顾的目的。无论是采用住房消费补贴还是住房供给补贴,都应当增大供给弹性,减小需求弹性。 相似文献

China has recently unveiled an ambitious new health-care reform plan, entailing a doubling of government health spending as well as a number of concrete reforms. While the details of the plan have not yet been completely announced, we offer a preliminary assessment of how well the reform is likely to achieve its stated goal of assuring every citizen equal access to affordable basic health care. The reform is based on three fundamental tenets: strong role of government in health, commitment to equity, and willingness to experiment with regulated market approaches. Within this framework, the reform offers a number of laudable changes to the health system, including an increase in public health financing, an expansion of primary health facilities and an increase in subsidies to achieve universal insurance coverage. However, it fails to address the root causes of the wastes and inefficiencies plaguing China's health care system, such as a fragmented delivery system and provider incentives to over-provide expensive tests and services. We conclude that China should consider changing the provider payment method from fee-for-service to a prospective payment method such as DRG or capitation with pay-for-performance, and to develop purchasing agencies that represent the interests of the population so as to enhance competition. 相似文献

Employee representation is an important aspect of the employment relationship. In some countries (e.g., Germany), the government mandates employee representation. In other countries (e.g., the Commonwealth), employers voluntarily recognize employee representatives. In the United States, employees must earn their right to be represented in a certification process that results in collective bargaining. With the significant reduction of union membership in the United States, this representation model is in decline. Other forms of participation have not picked up the slack. Hence, there remains a significant representation gap. In this paper, I describe and analyze several solutions to the representation gap.
