手术治疗急性梗阻性大肠癌临床探讨(附168例报告)

目的 探讨急性梗阻性大肠癌的手术治疗原则及预后.方法 分析168 例急性梗阻性大肠癌的手术治疗临床资料,其中右半结肠癌18 例,左半结肠癌60 例,直肠癌90 例.168 例中Dukes B 期24 例,C期80例,D期64 例.168 例均手术治疗,其中行结肠造口18 例,miles 手术64 例,Hartmann 手术18 例,一期切除吻合加肠外置术12 例,结肠灌洗一期切除吻合56 例.结果 治愈164 例,死亡4 例.未能切除癌肿的2 例患者术后5 月死亡.结论 手术治疗是解除急性梗阻性大肠癌的唯一方法.应根据患者的不同情况选择合理的手术方式.

Surgical treatment of acute obstructive colorectal cancer clinical discussion(with 168 cases of report)

Objective Explore the principles of surgical treatment and prognosis of acute obstructive colorectal cancer.Methods Analysis of 168 cases of surgical treatment of acute obstructive colorectal clinical data,In the 84 cases,right hemicolonic cancer occurred in 18 cases,left hemicolonic cancer in 60 cases and rectal cancer in 90 cases.Of the 168 patients,24 cases were classified as Ducks B stage,80 C stage,and 64 D stage.All these 168 cases underwent surgical treatment,of which 18 cases were subjected to colostomy,64 to Miles’ operation,18 to Hartmann’s operation.12 to primary resection and anastomosis plus intestinal exteriorization and 56 to primary resection and anastomosis plus colonic lavage.Results 164 cases were cured and 4 died.The 2 cases from failure in resection died in five months later after operation.Conclusion Surgery is the only method of treatment of acute obstructive colorectal.According to the different situations of patients should choose reasonable operation method.
